Supervision des pratiques:  

Objectifs:  
Le contenu de cette supervision s'appuiera sur le respect de la mise en oeuvre concrète des interventions relatives aux dernières bonnes pratiques professionnelles de la Haute Autorité de Santé à travers d'une étude de cas ou d'observations/interventions sur le terrain.

Appui, régulation et supervision des équipes dans l’analyse et l’interprétation des spécificités des personnes, des hypothèses causales des troubles du comportement ou de la mise en œuvre des plans d’actions propres aux projet personnalisé ou de la CAA. 

Soutien afin de mettre du sens dans la mise en œuvre des actions visant à réduite l'impact des troubles du comportement à partir du principe des évaluations fonctionnelles, leurs suivis et permettant de développer la communication concrète. 
Démonstration de gestes techniques ou régulation des pratiques dans le cadre du suivi des actions. Monitoring des actions et interventions en lien avec le terrain. 

Objectifs opérationnels : 
Cette démarche de supervision s’inscrit donc également dans la démarche d’amélioration continue de la qualité des ESMS (démarche d’évaluation interne et évaluation externe). Elle doit accompagner et soutenir le suivi du plan d’amélioration de la qualité élaboré à partir des résultats des évaluations, et en supervisant des actions précises à mettre en œuvre et leurs suivies. Ce soutien aux équipe et cette supervision s’inscrit dans le cadre d’une représentation multifactorielle de la personne et selon différents domaines d’évaluations.

Cette Supervision et appui au sein des équipes de professionnels s'adressent aux établissements accueillant des personnes ayant une déficience intellectuelle, avec ou ,sans TSA et présentant des troubles associés.  

En voici les thématiques:  
•    L’évaluations des compétences des professionnels quant à l’identification des besoins de la personne et selon une perspective globale du fonctionnement, et à l’aide d’outils standardisés et recommandés. 
•    L’adéquation des pratiques et des prestations proposés aux réels besoins identifiés de la personne et  dans le cadre des outils d’évaluation de l’analyse fonctionnelle ( évaluation des compétences adaptatives de la personne ( Vineland), de son niveau d’abstraction  et de compréhension ( Com voor), de sa douleur ( grille GED-DI), de son profil sensoriel etc… et au profit de l’analyse multimodale.
•   ou encore de la capacité des professionnels à formaliser
 les hypothèses, les tracer, et leur apporter un suivi  ainsi qu'aréévaluer celles-ci dans le cadre de l’analyse fonctionnelle et multimodale des comportements problématiques.  Et des plans d’actions définis dans le cadre de la boite à outils des méthodes ( TEACCH, ABA, PECS, Montessori, Stimulation basale…)
•    L’accompagnement et le suivi et de la mise en œuvre des plans d’actions individualisés, 
•    De la mise en œuvre de la communication concrète et selon les recommandations préconisées par l’HAS et pour développer le plan individuel de la communication selon les principes de la CAA.

Infos pratiques:

Supervision et soutien des équipes éducatives au sein de l’établissement et en intra-muros et en lien avec une ou plusieurs étude(s) de cas.

Tarif hors frais de déplacement et hébergement du Superviseur: 1250 euros/journée (Exonération de TVA conformément à l'article 261-4-4° du code général des impôts) avec un minimum de 3 jours dans l’année et qui peut être jumelées avec plusieurs journées pour la même association ou plusieurs unités.  

Intervenante : M.ALLEGRE Psychologue et ancienne Chef de service éducatif.          Neuropsychologue, experte en TSA et dans l'accompagnement des personnes ayant un TDI. Orthophoniste spécialisée en troubles du comportement et CAA.

Modalité pédagogique et d'intervention  : Supervision en présentiel

Supervision  destinée aux établissements et à tous les professionnels accueillant des personnes présentant une déficience intellectuelle ou des troubles du développement, des personnes présentant des Troubles du Spectre de l’Autisme ou polyhandicapées. 

Délai d’accès : A partir d’un mois après l’entretien d’analyse des besoins. Nous envisagerons la date de début et l’organisation la plus appropriée en fonction de vos besoins préférences ou contraintes. Nous sommes en capacités de  débuter l’action de formation sous un mois.

Modalités d’accès: En amont de la contractualisation, un entretien téléphonique et un questionnaire permettra d’évaluer et analyser les besoins afin d’ajuster le contenu aux profils et situations des stagiaires. Celui-ci pourra également faire l’objet d’ajustements en amont et durant la supervision en lien avec les attentes exprimées. 

 Horaires indicatifs : 9H-16H30 ( le cas échéant et en fonction des besoins 17H).

  • Practice supervision  (see below) in the field of support for people with intellectual disabilities, autism, individualized recommendations, functional analysis and causal hypotheses of behavioral disorders with institutions but also mobile autism teams. From the work with people with ASD or with an intellectual disability, the supervisor helps to assess their interests and preferences, to set up the bases of an alternative communication, to determine the methods of evaluation / management of a problematic behavior.

  • Support to respond to calls for tenders from the ARS (having participated and won in 2017 the call for tenders concerning the device of the DATSA mobile team in Burgundy and mandated by the ADFAAH, and in 2021 the project allowing the (positive inclusion and employment for high-level ASD people mandated by EURECAH in the Rhône-Alpes Auvergne region Loire region, and support for many other ongoing projects) .   • Support for the drafting and development of a project for the opening of  innovative structures or expansion projects intended for the reception and support of people with Autism or ASD. Customizable quote on request.  

  • Updating of the establishment plan and institutional tools based on the needs of people with autism. Customizable quote on request and tools offered as part of the construction partnership with the project teams.  

  • Expertise and advice in the context of architectural and space planning based on the needs of people with autism and the 3rd plan Autism and 4th plan.

Follow-up program for the actions and supervision of teams working in a medico-social structure as part of supporting people with intellectual disabilities and serious behavioral disorders.

 

  Practical information:

Intramural training for all professionals from ESMS or associations, working with people with intellectual development disorders with or without autism and problem behaviors. 

Minimum of 3 days during the year at the rate of one per quarter and possibility of being able to organize a day every two months. 

Price excluding trainer's travel and accommodation costs: € 1,200: days of supervision.  


Goals :  

  Supervision, monitoring and support within teams of professionals and establishments welcoming people with intellectual disabilities and with autism spectrum disorders in:

-  Assessment of the person's skills and needs from a global perspective of functioning,

- Support and monitoring and implementation of individual action plans,

- The implementation of concrete communication and according to the recommendations recommended by the HAS,

- or the functional and multimodal analysis of problematic behaviors.


This supervision is thus in line with the objectives set out by the General Directorate of Social Cohesion, including INSTRUCTION N ° DGCS / SD3B / CNSA / 2015/369 of 18 December 2015 relating to the development of the medical offer. social service welcoming and supporting people with autism spectrum disorders, and which specifies that:  

The application of the recommendations of ANESM's Good Professional Practices and the 2013/2017 autism plan, in its action sheet n ° 6, provides for specific means for the "strengthening by transformation of the existing medico-social offer, with regard to recommendations of good practices in force ”, in order to take into account the specific nature of the support for people with autism spectrum disorders (ASD), in particular in terms of team composition, training, supervision of professional practices , nature and pace of interventions.   

In this sense, our actions are therefore strongly based on the recommendations of good professional practices produced by the National Agency for the Assessment and Quality of Social and Medico-Social Establishments and Services (ANESM) and the Haute Autorité de Santé. (HAS):

• June 2005: “Autism screening and diagnosis” (Childhood), French Federation of Psychiatry, in partnership with HAS;

• January 2010: “For quality support for people with autism or other pervasive developmental disorders”, ANESM;

• January 2010: “State of knowledge”, HAS;

• July 2011: “Diagnosis and evaluation of autism and other pervasive developmental disorders in adults”, HAS;

• March 2012: “Autism and other pervasive developmental disorders: coordinated educational and therapeutic interventions in children and adolescents”, ANESM-HAS;

• Under development: interventions and personalized project for adults (ANESM-HAS). These aim to disseminate and improve knowledge of the particularities of people with ASD, methods of assessing their skills and difficulties and recommended methods of intervention. They provide benchmarks for setting up an appropriate organization of support and the necessary professional skills.

• The 2017 Séraphin-PH Work Program, which reformed the pricing of medico-social establishments and services for disabled people, was validated by the strategic committee of February 9, 2017, chaired by Ségolène Neuville, Secretary of State in charge of People with disabilities and the fight against exclusion

Ways :
 

Support and supervision of teams in the analysis and interpretation of the specificities of people, the implementation of action plans specific to personalized projects, the implementation of means to be developed and in order to improve their level of comfort , their interests or preferences and concrete communication, or the causes of serious behavioral disturbances.

This supervision thus proposes to accompany and support the teams in the overall assessment of the person's functioning, the follow-up of actions and the functional analysis based on the needs of the people. Its objective is to supervise and support the teams in the implementation and interpretation of observations, evaluation tools and professional practices in accordance with the recommendations cited above.  

 

This support and supervision starts from the needs of users while respecting the recommendations of ANESM's good professional practices and the application of the 3rd and 4th Autism plan or the support tool for the evolution of the offer of the '' support and interventions within the framework of practices with beneficiaries (Implemented by the General Directorate of Social Cohesion). It therefore follows the implementation of practices in the field of intellectual disability and associated pathologies of professionals by ensuring that they are in line with the needs of people and tools used in the field of functional analysis. In addition, it wishes to be able to support the various training courses already undertaken in this direction within the requesting establishment itself. 


It is therefore a question of supporting and highlighting the acquisition, maintenance or improvement of knowledge in the context of mastering the interpretation of observations, personalized recommendations and in the field of intellectual disability or autism, data or multimodal analysis tools and finally functional assessment tools in order to have a multifactorial representation of the person and according to different areas of assessment.
This supervision approach is therefore also part of the process of continuous improvement of the quality of ESMS (internal assessment process and external assessment). It must accompany and support the follow-up of the quality improvement plan drawn up from the results of the evaluations, and by supervising the specific actions to be implemented and their follow-up. 
 
Methods of intervention: 
The supervision of practices aims to support professionals in the implementation of the content covered in theoretical training. It is to be distinguished from the analysis of practices. It involves:

- Demonstration of technical gestures: The supervisor sets up in front of the professionals, the techniques and procedures relating to the approaches taught in training, which allows a better appropriation of them.

- Active observation: The supervisor observes the members of the team implementing the techniques and strategies with the people and brings them an immediate comment able to correct errors and approximations.

- The objectification of observations and identification of people's needs using analysis tools (Excel file for ABC analysis), data collections and tests or evaluations recommended for this audience (Cars, Vineland, Com Voor, sensory profile) and by the HAS.

- Support in the practice of in-depth psychological assessment of the person.

- Organizational support: Reflect with the management team on the logistical and organizational means involved in the recommended techniques (distribution of tasks and resources, identification of needs for dedicated time and material preparation, planning of learning times , organization of spaces, implementation of “daily” assessments: ratings and data grids, etc.).

- Individualized recommendations: From work with people with ASD, the supervisor helps to assess their interests and preferences, to set up the bases of an alternative communication, to determine the methods of assessment / management of a behavior problem.

 

The supervisor's scope of action potentially covers the implementation (or support):

- evaluation tools and methods of drafting the individualized support project;  

- techniques relating to the support of people starting from  assessment of the needs of the people received and standardized or recommended tools.  

- the spatio-temporal structuring of the environment;

- learning strategies: decomposition into sub-tasks, guidance / blurring of guidance, shaping, generalization and maintenance of skills
- alternative communication tools;

- functional assessment procedures and management of problem behaviors.
 
This supervision action will take place over days to be defined with the supervisor and the intra-establishment establishment (and if requested with an additional videoconferencing option for regular monitoring of actions) through:
 
• Time for discussion and coordination or the development of supervision in partnership with the management team.
 

• Observations of practices, assessment tools, arrangements in the field of structuring, action plans or communication tools and / or problems of residents chosen for supervision work.

• A time for meetings and coordination of proposals and clinical tools of functional analysis in partnership with the paramedical team in order to support the actions while starting from the needs of the people. These people are of course recognized and considered as resources in the field of supervision and the quality process. Supervision is thus offered in order to strengthen support for these proposals (and not allow the team to weaken or replace the quality of the work undertaken in this direction).  

• Analysis of the actions carried out and reporting between each supervision of the data collection carried out by the professionals, as well as their initial analysis in this direction or action or improvement plans and in the face of problematic situations that some users may present.  

• And direct supervision work to develop the follow-up of actions and the process with teams in multi-professional meetings and which will allow the supervision of actions in partnership with  the management team in the same dynamic as we described previously in our objectives.
